The SET pin is sensitive the leakages and disturbances. The capacitance on the SET pin to GND and DIV pin to GND should be less than 10pf. I recommend separate voltage divider source for MOD input. I think that changing the ramp profile or using the 6992-4 (with some disable circuit) is the way to go to prevent the very short pulses.
